The Parish Center Kitchen is needed
Food will be sold.
Alcoholic beverages will be served/sold.
This is a fund raising event open to the public.
Video equipment is needed.
A microphone is needed.
A Dry Erase Board is needed.
The event is reserved for the Gym or Benedict Hall and needs:
a. Special seating arrangements
b. Rectangular tables (4,6, or 8 ft)
c. Round tables, large or small
d. A microphone

Remember: The parish has a limited number of tables, chairs and equipment. They will be provided when available.

Bulletin Submission Form – for requesting printed announcements in the weekly bulletin. Due noon on Friday 9 days before the Sunday you would like your announcement to appear. [also available as a PDF]
Calendar/Room Reservation Form [June 2008] – to get your event approved and added to the parish calendar, to reserve a room or to requesting other parish support. Due three weeks before your event.
Check Request Form – for requesting reimbursement by check for approved expenses by the office. [also available as a PDF]
Report to the Parish Council Form – for Council Representatives and Pastoral Team members to report on their areas each month. Due at the start of the Parish Council meeting. [also available as a PDF]
Change of Name and/or Address Form – for already-registered parishioners who want to change how their name or address appears in parish records. [also available as a PDF] (Note: new parishioners need to register in person in the parish office)
Tithing Application Form – Request a tithe (10 percent of a regular Sunday collection) for your organization; applications due by the end of March or September of each year.
